GP practices work in groups called Primary Care Networks. Your practice is in Swaffham and Downham PCN which comprises 7 practices in the Swaffham and Downham Market areas. They include:
- Litcham Health Centre
- Plowright Medical Centre
- Manor Farm Medical Centre
- Campingland Surgery
- Bridge Street Surgery
- Howdale Surgery
- The Hollies Surgery
In order to improve access to general practice services, and promote good health for all the population, GP Practices across the country have been asked to offer more appointments to patients, across the PCN, outside of their current core opening hours of 8am and 6:30pm. Swaffham and Downham Primary Care Network will be providing these services from 6:30pm to 8pm weekdays and 9am to 5pm Saturdays.
Clinics will take place remotely and face to face in one of the 7 practices mentioned above and may be performed by a range of clinicians such as paramedics, pharmacists, care coordinators, mental health practitioners, physiotherapists, nurses, GPs and health and wellbeing coaches.
Patients will either be given the option to book onto one of these clinics or be invited/contacted by the clinicians directly.
Please bear in mind that some of these services will be provided remotely and as such you may receive calls from numbers outside of area or withheld.
Our clinicians will never ask you for financial information, as their sole purpose is to provide you with a health service.
Please do report to the surgery any concerns regarding these calls/visits so that we can investigate and appropriately deal with any concerns.
